9 y/o male presented to the Peds ED with a foreign object in his foot. It went through his shoe and caused a small puncture of his foot. How should we handle this? What kind of wound care should be provided? What should we look for on Physical exam? Should we start Abx? If so, what should we cover for? Are there any other prophylactic medications to consider? And as a bonus...what the heck is this thing that was in his foot? Answer to follow tomorrow - and if you've already heard this story from Dr. Izsak or myself please don't spoil it!

Take it out, it is not part of the foot. What kind of wound care should be provided? exploration, irrigation. What should we look for on Physical exam? r/o signs of infection; erythema, pustular discharge, streaking, gas, bone injury (get x-ray, u/s picks up other non metallic objects better)Should we start Abx? If so, what should we cover for? went thru shoe, cover for psuedomonasAre there any other prophylactic medications to consider? i'd consider lots, but settle on oneAnd as a bonus...what the heck is this thing that was in his foot? We couldn't rule out that it was potentially a bone from a mammal and along that line it may potentially be from an animal that may have rabies. So Dr. Iszak elected to start the kid on rabies shots. I posted the photos of the object on Facebook and asked my friends if anyone knew what that thing was. In about 30 minutes we had our answer, a pectoral spine from a catfish. So the kiddo doesn't need to complete the rest of his rabies shots, just the antibiotics for pseudomonas coverage.
