When looking at kids with murmurs whats everyones approach. My understanding has been that if theyre well appearing found incidentally and have no exertional components of fam hx of death fine for outpt. However Ive seen some variation when sick. So you have febrile and heart murmur, some admit all and want echo r/o endocarditis, some take into account type of murmur, as with fever theres increased CO and some of the murmurs are second to that. Thoughts?

murmur + minor unrelated complaint = home
murmur + possibly related complaint usually gets admitted (or at least talk to peds cardiologist)
as a side note, sometimes talking to the pediatrician will give some insight (i.e. pediatrician already knows about it)

in my opinion it depends. sometime murmur can be 2nd to tachycardia whit fever. if the kid looks good and parents are reasonable i would be ok sending them home if the PCP is on board
