Case from today. 5 week old cough, nasal congestion, multiple normal axilallry home temps and 1 @101.0. no meds given. pt normal birth history had initial immunizations, no med problems, drinking less, making normal wet diapers, no respiratory distress, or other complaints. URI sick contacts in house. Kid arrive has rectal temp 99.0, normal physical exam, fully exposed, no rashes, lungs clear, abd benign, fontanelles non bulging. Recheck rectal temp 98.8 PCP will see him in 36hr
Do you do full workup on normal appearing kid based on positive home fever, or forgo workup due to normal exam and 2 documented negative rectal temps with follow up within 36 hrs? The main question being, dose a home documented fever (regardless of location) trump a negative rectal temp in ED when no meds were given?
