Weve hit a period of debate for tPa in submassive PE. It seems studies tend to be quite split on whether it is beneficial in the pt who doesnt have massive pe(shock). Heres a recent study that looked at using a smaller dose ie "safe tPa" for the submassive PE. Randomization looks very good between two arms, with statistically significant improvement in those who got tPa. emcrit.org/wp-content/uploads/2012/12/Sharifi_MOPETTstudy.pdfSomething worth following going forward as we may begin giving tpa in large PE burden with signs of pulmonary hypertension. Dr Doyle at TTH is one to talk to about this as it seems things are done along this pathway in Detroit Lastly will someone for the love of god start using this site again, its becoming a cast away situation where I just sit here and talk to myself
