I have shown this case to a few of you in Toledo's ER so please dont share the answer just yet. 54 year old male, drinks 30 beers a day, smokes, has HTN but does not take his meds, has not see a doctor in 15 years, and has not been to any other ER recently. Comes in with epigastric pain that radiates to his back and bloos in his stools for 2 days. he is vomiting, but denies any blood in his vomit. He says his stools are black and sometimes mixed with dark red blood. on exam his is tachycardic and hypertensive, has moderate epigastric tenderness with guarding and rebound. attached is a CT with IV contrast only - due to vomiting and high BMI - radiologist says abnormal appreance of proximal small bowel which could be due to infection, inflmation or ischemia, "please correlate clinically" What do you guys think about the CT? IS there anything concerning that was not mentioned by the radiologist? Attachments:

correct. you can see a large amount of dye within the intestines but remember this was done with iv contrast only and he had not gotten any PO contrast. turns out he had a chronic thrombus in his SMA that caused high pressures within the vessel causing an aneurysm which slowly eroded into the proximal intestine causing him the bleed very rapidly into his intestines. his hg went from 12 to 5 in 4 hours time.

Can you see the thrombus? Hard to tell based on a single slice but it looks like it to me. Aorto-enteric fistula was def high on the list based on the history.

Stransky-you can't see the thrombus directly on the ct scan but there was a lot of 2nd signs like edema and fat stranding in the distribution of the SMA which is bit in the attached image.
On side note you two should have old school brake dance -dance off to settle any argument.
