great topic, weve been hearing him pump this on emrap and emcrit for a few months now, seems like a great way to safely intubate a pt who would otherwise start at ~90% and quickly drop after. The other 2 things ive seen that have some impressive results involve apneic oxygenation. 1 group used a nasal trumpet hooked to the O2 and bough time, the other taped O2 tube to mac blade and delayed desat by an exra 2+ minutes

Interesting paper but I still have a few questions. Their primary outcome was desaturation. I would also like to know fail rate of first attempt. Also what are rates of aspiration or VAP.
It's a cool topic and I will try it in practice but would like to see more data from a much larger sample size.
Also did you notice that some of these patients were at 60-70% before they started this protocol. That takes some guts.

I meant to talk about this paper in journal club this week. I think this whole concept is meant for those people who are unable to be preoxgenated by regular methods. That's probably why they all started at a lower sat. I would think the fail rate of first attempt would be lower just because it gives us a chance to be more prepared and take a look before we paralyze them. But I would like to see the data too.
