21 yo male i recently saw in Cambridge Ohio. no PMhx, has no doc, takes no meds does not drink or smoke. initally seen in the fast track for 4 days of viral like complaints: cough, fever, body aches, runny nose, fatigue, ect. he didi have a fever and was tachy in the FT and the doc there did CXR, flu swabs, one liter bolus and 1 gram of rocephin IV (no idea why). his meds/fluids were still running at the end of her shift so she put up his paper work and told nurse to DC when done. Meds and fluids were done and his nurse was worried because he was still tachy and febrile and did not feel comfortable discharging so brough him over to me in the main ED. he looked to me like all the other viral stuff going around but still febrile and tachy. told me he was having episodes of palpitations and near syncope as well. i started a further workup including EKG (which is attached). chek out his EKG and let me know what you think and what you would do. He looks like brugada syndrome, admit to cardiology. If he's tachy and near syncopal this could degrade easily into a fatal arryhtmia. Needs beta blockers in interim

indeed. type one brugada or my "holy grail" of EKGs. faxed it to EP guy who agreed.

Big thing for this was an article I was reading about use of beta blockers when febrile or tachy, not isoproteronol which is theusual treatment, due to increased risk of v fib when theyre febrile

attach article, please. interested to see methods and numbers as i imagine data for type one brugada patients is hard to come buy. i'll give this paper the bruss-over
