20 yo man brought to the er by his mom for a "second opinion". the day before he passed out and hit his head. went to another ER, they scanned his head and neck and sent him home. on history he passes out frequently for the past year. prodrom of dizzy, sob, heart palpitations then syncope. no post ictal state. has not seen a doctor for it. when he was younger he had bad sob while playing sports, passed out once durring sports and was diagnosed with sports induced asthma. he is reliable, takes no drugs, etoh, cigs. he is in nursing school. exam- has an abrasion on left forehead but everything else (heart, lungs, abd, neuro, ect) is all normal. his mom, who is not pushy or anoying, says that the patients dad dropped dead all the sudden in his late 30's. after i asked her to call her husbands side of the family, it turns out out of the fathers brothers and uncles, only one lived past 45. all the others "dropped dead" but no one ever knew why. attached is his EKG. what is your interp and plan for management? Being 20 with hx I say brugada type 1. I was thinking HOCM initially but there is a pseudo rbbb with the start of a biphasic t in v1 and a bit in v2.

I'll put this on both places.
EKG #2 definitely looks like a little hump on the ST consistent with Brugada more than the 1st. I can only think that leads were moved slightly closer together to get better amplitude but I don't see evidence of lead reversal from #1 to #2. I don't really see dagger like S waves for HOCM, Intrigued to hear what it was. I can tell you with the history the kid stays and sees Cardiology even with normal EKG.

spot on, i moved the precordial leads up a fec CM, which can pick up a brugada not seen on the first ekg. on the second one there is more of an incomplete right bundle with slight st elevation in V1 and V2. not slam dunk but concerning. EP guy argreed. so far normal stress echo and cardiac MRI. still no definitive answeres will keep you posted
